How to fill out this Special Request Form

Step 1. Search the Archives of Ontario Microfilm Interloan Catalogue to identify the reels of microfilm or fiche that you would like to order.
Step 2. Note the reference code and a brief description of the records.
Step 3. Enter the microfilm reels/microfiche needed in the fields below.
Step 4. Click Submit.
